Cognitive Security: The Attack Surface Nobody's Watching

Most organisations thinking about AI security are focused on the model. Is the model safe? Is it biased? Can it be jailbroken? Those are real concerns. But they're not the biggest risk. The biggest risk is the human using the model - and the chain of vulnerability that starts with one person's poorly structured prompt and ends with sensitive corporate data sitting in a training pipeline you don't control.

What cognitive security actually means

Cognitive security is a category we've named and defined at Galahad. It refers to the human vulnerability chain in AI use - the organisational attack surface that exists between the person typing a prompt and the business outcome that prompt influences.

It's about specific, exploitable weaknesses: employees pasting confidential data into AI tools, teams building critical workflows on unaudited prompts, decision-makers trusting AI outputs without understanding how those outputs were generated, and entire organisations exposing their strategic thinking to systems that may be logging, training on, or leaking that information.

Traditional cybersecurity protects systems. Cognitive security protects the thinking that happens when humans and AI systems interact.

The vulnerability chain

The chain is predictable and it follows the same pattern in almost every organisation we've assessed.

Stage one: individual exposure. A single employee uses an AI tool - often a free tier, often without IT's knowledge - and pastes in something they shouldn't. Client data, financial projections, strategic plans, legal documents. The data leaves the building. Nobody notices.

Stage two: workflow dependency. Teams start building processes around AI tools without formal evaluation. Prompts become institutional knowledge - but unversioned, unaudited, and maintained by whoever happened to write them first. The quality of critical business outputs now depends on prompt engineering that nobody owns.

Stage three: decision contamination. Senior leaders start making decisions based on AI-generated analysis without understanding the provenance. Where did the data come from? What assumptions did the prompt encode? What was excluded? The AI becomes an invisible co-author of strategic decisions - but it's not on the org chart and nobody's reviewing its work.

Stage four: corporate compromise. The combination of exposed data, unaudited workflows, and unexamined decision inputs creates an attack surface that traditional security tools don't monitor, traditional governance frameworks don't cover, and traditional risk assessments don't capture.

What an AI pen test looks like

A cognitive security assessment maps the full vulnerability chain across four dimensions.

First, data exposure: where is sensitive information leaving the organisation through AI interactions? This includes sanctioned tools, unsanctioned tools, and the grey area in between - the personal ChatGPT accounts, the browser extensions, the Slack integrations that nobody formally approved but everybody uses.

Second, prompt integrity: what prompts are driving critical business outputs, and who controls them? We audit the prompts that generate reports, analyse data, draft communications, and inform decisions. We check for injection vulnerabilities, bias encoding, and hallucination triggers.

Third, decision provenance: which decisions are being influenced by AI, and can the organisation trace the chain from input to output? When a board paper includes AI-generated analysis, can you identify what data went in, what assumptions were encoded, and what was excluded?

Fourth, organisational posture: does the organisation have the governance structures, policies, and training to manage cognitive security as an ongoing concern - not a one-off audit?

Why this matters now

Twelve months ago, cognitive security was a theoretical concern. Today it's an operational one. The adoption of AI tools has outpaced every governance framework designed to manage them.

Most organisations are in stage two or three of the vulnerability chain right now. They have employees using AI tools with sensitive data. They have teams building workflows on unaudited prompts. They have leaders making decisions based on AI analysis they can't trace.

The regulatory environment is catching up - the EU AI Act, emerging UK frameworks, sector-specific guidance from financial regulators - but regulation follows incidents, and the incidents are already happening. The organisations that address cognitive security proactively will have a structural advantage over those that wait for a breach to force the conversation.

This isn't about fear. It's about the same principle that drives everything at Galahad: map the risk surface before you build. AI is too valuable to avoid. It's also too powerful to use without understanding the vulnerability chain it creates.
